CASE MANAGEMENT SERVICES

Our case managers serve as your friend who can help guide you through the maze of available services. They help you or your family obtain assessments, develop plans of care to meet individualized needs, and coordinate services. They make frequent visits and maintain close communication to ensure your needs are being met.

POSITIVE BEHAVIORAL SUPPORTS / PSYCHOLOGICAL SERVICES

A behavior specialist is a licensed counselor who assist you or your loved one find solutions to acting out behaviors. They establish the cause and effect of the behavior and work closely with the individual and caregivers. With these service(s), staff coordinates supports to meet the mental health needs of individuals by conducting individual, family and group therapy to ensure adequate mental health services are available.

RESPITE

Providing periodic relief for family or primary caregivers that are responsible for care and supervision of the individual at the same home. The service is provided in the individuals residence or in an out of home setting.

RESIDENTIAL SUPPORTS

Residential Supports provide a home like setting to support individuals to live independently and to have a choice over their lifestyle as residential support staff work directly with individuals to perform tasks such as laundry, routine house hold care, self care, socialization, and other basic activities.

ADULT DAY TRAINING (ADT)

Assisting with self-help skills, social skills, employment skills training and behavior modification Working with intellectual or developmental disabilities who may be behaviorally challenged and supporting the recipient in daily and meaningful routines in the community. Focusing on activities of daily living, self advocacy, adaptive and social skills, and vocational skills. Adult Day Training is offered onsite or offsite and great for persons as a transition process from school.

COMMUNITY LIVING SUPPORT

This service allows staff to assist the individual with routine household tasks, activities of daily living, personal hygiene, shopping money management, medication management, socialization relationship building, leisure choices, participation in community activities, therapeutic goals and non medical care.

PERSONAL ASSISTANCE

Provides support and training working directly with individuals in their own homes to help them perform whatever tasks are necessary for successful implementation of their Individual Support Plans.
